Be the Connection — Case Manager

Brain Injury Connections of the Shenandoah Valley is hiring a Case Manager/Brain Injury Support Specialist to provide person-centered services to individuals and families living with brain injury across Page, Shenandoah, Rappahannock, and Rockingham counties.

 

This is a hands-on, relationship-driven role that blends traditional case management — intakes, assessments, service planning, and resource coordination — with direct, one-on-one support to help clients build independence and thrive in their homes and communities. Regular travel throughout the region is part of the job, and so is being part of a small, passionate team doing meaningful work every day.

 

We’re looking for someone with a bachelor’s degree in Social Work, Human Services, Rehabilitation, or a related field (master’s preferred), strong communication and organizational skills, and a reliable vehicle. Experience with brain injury or case management is a plus, but we’ll train the right person. This is a 24 hour per week position. Salary range is $28,000–$30,000. Bilingual candidates are strongly encouraged to apply.
